A Direct Bilirubin test measures conjugated (direct) bilirubin, the form that has been processed by the liver. This test helps identify whether elevated total bilirubin is due to liver/bile duct problems or red blood cell breakdown.
What This Test Measures
Direct (Conjugated) Bilirubin:
After the liver processes bilirubin from red blood cell breakdown, it becomes water-soluble (conjugated) and can be excreted in bile. Elevated direct bilirubin indicates the liver is processing bilirubin but it can’t be properly excreted.
Reference Range
Normal: 0.0-0.3 mg/dL
Who Should Consider This Test
• Elevated total bilirubin requiring further evaluation
• Jaundice (yellowing of skin/eyes)
• Dark urine with pale stools
• Suspected bile duct obstruction
• Monitoring liver disease
• Evaluating newborn jaundice
What Elevated Direct Bilirubin Indicates
Liver Disease:
• Hepatitis
• Cirrhosis
• Drug-induced liver injury
Bile Duct Obstruction:
• Gallstones blocking bile duct
• Pancreatic cancer
• Bile duct cancer
• Strictures
Direct vs. Indirect Bilirubin
• Elevated Direct: Liver/bile duct problem (obstruction)
• Elevated Indirect: Increased red blood cell breakdown (hemolysis)
• Both Elevated: Liver disease affecting processing and excretion
