What happens next:
Once our clinic receives your medical records from Elevate MD, one of our staff members will be in contact with you to schedule your consultations with our Physicians to review your current medications to move forward with medication refills.
When can I get scheduled?
As soon as we get your records from your previous provider, we will work on transferring your records into our system. Once this has been completed, a member from our team will contact you to get you scheduled.
How much does this cost?
This will all depend on your specific protocol. Our monthly payment plans include your prescriptions, follow-up lab work, and appointments moving forward.
Do we take insurance?
We do not take insurance at this time.
