An Indirect (Unconjugated) Bilirubin test measures bilirubin that has not yet been processed by the liver. Elevated indirect bilirubin typically indicates increased red blood cell breakdown or conditions affecting liver uptake.
What This Test Measures
Indirect (Unconjugated) Bilirubin:
Bilirubin produced from hemoglobin breakdown before it reaches the liver for processing. This form is not water-soluble and is carried in the blood bound to albumin.
Reference Range
Normal: 0.2-0.8 mg/dL (calculated as Total Bilirubin minus Direct Bilirubin)
Who Should Consider This Test
• Elevated total bilirubin requiring differentiation
• Suspected hemolytic anemia
• Evaluating Gilbert’s syndrome
• Jaundice workup
• Newborn jaundice evaluation
What Elevated Indirect Bilirubin Indicates
Hemolysis (Red Blood Cell Destruction):
• Autoimmune hemolytic anemia
• Sickle cell disease
• Transfusion reactions
• Hemolytic disease of newborn
Gilbert’s Syndrome:
• Benign genetic condition affecting 3-7% of population
• Mild, intermittent jaundice
• No treatment needed
Other Causes:
• Ineffective erythropoiesis
• Large hematoma reabsorption
• Some medications
Direct vs. Indirect Bilirubin
• Elevated Indirect: Problem before liver (hemolysis) or with liver uptake
• Elevated Direct: Problem with liver processing or bile excretion
• Both Elevated: Liver disease affecting multiple steps
